New Dental Patient Info
At your first visit, our goal will be to gather information about your current oral and overall health while learning more about your long-term goals for your mouth. While we’re examining your mouth, we may ask you questions about any issues you may have noticed recently. Then we can review the information we’ve gathered and give you our recommendations for treatments that you might need in the near future. You may ask us questions at any time if there’s something you don’t understand.
Payment Options
At The Center for Advanced Dentistry we make every effort to provide you with the most convenient financial options. Please bring your insurance information with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our front desk staff. For your convenience we accept Visa, MasterCard, Discover, American Express and CareCredit. Payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 216-595-1710.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Please ensure the insurance information you have provided is accurate so we can process your claims and utilize your benefits. It is the patient’s responsibility to provide the correct information.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame.

We also regularly work with CareCredit, which is a third-party financing company that offers lines of credit at low or no interest to help patients obtain the medical and dental healthcare they need.
Apply for CareCreditCancellation Policy
Your appointment time is reserved specifically for you and for you only. Because of this, missed appointments or late cancellations are extremely detrimental to our day. As a result, we request at least 24 hours advance notice if you will not be able to make your appointment. Patients that miss or cancel appointments with less than 24 hours notice will be charged a $55/hr broken appointment fee. If you think you will be late, please call as soon as possible so that we may advise you if your late arrival can be accommodated, or if we will need to reschedule you.
